EMPLOYEE KILLED IN FALL FROM FORKLIFT PLATFORM
Investigation abstract
On March 13, 1990, employees were installing an automated storage system consist ing of two 3-ft-9.125-in. by 119-ft metal racks, 31 ft 10 in. in height. An unse cured 48 in. by 89 in. wooden platform was placed on the forks of the Clarklift. Employee #1, along with 15 aluminum containers, was elevated to the height of 2 0-25 ft. Employee #1 fell head down and was killed when placing the aluminum con tainers in the openings of the metal racks. The platform and two of the four rem aining containers followed his fall. Employee #1 was not wearing personal protec tive equipment.
