Employee killed when caught between table and machine
Investigation abstract
At approximately 11:10 p.m. on September 7, 1989, Employee #1, a computer checke 1, who was underneath the storage table placing tickets on rolls, became caught between the storage table and the lowering device. She was killed. r, was called to the cut order machine computer terminal to replace the terminal operator while he took a short break. When she got to the station, Employee #1 began to print tickets and place them on rolls of carpet in the area. At the sam e time, a forklift driver was placing rolls of carpet by the tickets. The cuttin g machine operator was at the machine station and had lowered a new roll of carp et to the cutting table from the storage table, but the roll did not drop to the belt table. The machine operator left the station to push the roll onto the tab le. As she pushed the roll, the lowering device pneumatically closed. Employee #
