EMPLOYEE INJURED WHEN STRUCK BY LOAD FROM FORKLIFT
Investigation abstract
At approximately 8:45 a.m. on November 1, 1989, Employee #1 was walking down the middle of an aisle-way when a forklift hit him from behind. The driver stopped suddenly dropping the load of brake drums. Employee #1 tried to get out of the w ay, but his hand and arm were caught under some of the load. Two of his fingers and part of a third were amputated.
