Employee's head crushed in press
Investigation abstract
At approximately 4:15 p.m. on January 28, 1990. Employee #1 had loaded the prech es to this accident. arger on a Durand hot press. He proceeded to the control panel and activated the squaring carriage on the precharger. Apparently, Employee #1 stepped up onto a conduit approximately 40 inches from the floor either to straighten a panel or t o look at something else. The operating cycle was not stopped when this procedur e was conducted. Physical evidence indicates that Employee #1's head was crushed between the squaring carriage and a steel structural beam of the press. Paramed ics were summoned and responded from Riddle. Employee #1 was transported to Fore st Glen Hospital, where he was pronounced dead on arrival. There were no witness
