EMPLOYEE CRUSHED BETWEEN PALLET CARS
Investigation abstract
Employee #1 a supervisor was between two mold pallet cars trouble shooting a hau l back device which was located in the center of the mold pallet car track syste m. He straddled the haul back device with his back to the mold pallet cars. Anot her employee noticed the cars began to move and activated the emergency stop how ever, the cars continued to move. Employee # 1's leg was caught preventing his e scape and he was fatally crushed. A lockout procedure had been developed and all employees trained, but procedures were not followed.
