EMPLOYEE LOSES TIP OF INDEX FINGER-OPERATING A PRESS MACHINE

Investigation abstract

At approximately 12:35 a.m., on July 26, 1989, Employee #1, a punch and stamp pr ess machine operator, lost the tip of his left index finger while he was operati ng a Johnson Machine and Press Company Full Revolution Press. The press has Seri al Number 56472. He apparently was operating the press without the use of restra ints to prevent injuries while making punches.
