EMPLOYEE DOING MAINTENANCE KILLED WHEN CAUGHT INSIDE MACHINE
Investigation abstract
At about 7:50 a.m., Employee #1 was cleaning the underside of an S and S and S D ie Master machine, Company Number 203. The area in which Employee #1 was working measured about 16 sq. ft. While Employee #1 was working, the machine operator j ogged the machine, catching Employee #1 in the aluminum draw bar and pinning her between the aluminum draw bar and the shaft for the chain and sprocket wheels. Employee #1 suffered a crushed upper chest and a broken jaw and shoulder. She wa s hospitalized and later died from her injuries.
