EMPLOYEE INJURED WHEN REMOVING PLANKS FROM SCAFFOLD
Investigation abstract
On January 17, 1990, an employee was ascending and descending a 12-ft-high tubul ar welded frame scaffold while removing scaffold planks with dimensions of 2 in. by 20 ft by 16 ft. He was placing them on the outside outriggers for a fork lif t to transport them away. As he was removing the last plank, the scaffold pulled away from the building and toppled over. The employee landed on top of the pile of scaffold planks and was injured.
