Employee killed in fall from scaffold
Investigation abstract
Employee #1 was putting 2 by 12 planks on a scaffold 9 ft above ground. Apparent ly, he improperly placed a plank with a cleat on the end frame. When he put his weight on the plank, he and the plank fell to the sidewalk. He was killed.
