Event description
EMPLOYEE KILLED IN 10 FOOT FALL FROM PITCHED ROOF
Investigation abstract
Employee #1 and another employee were stacking bundles of shingles on the roof o f a house. The employees had just completed loading between thirty-five and fort y bundles onto an eight-to-twelve-pitch roof by laying them in a row against a " two by four" along the pitch of the roof. The "two by four" pulled lose or broke causing both employees to fall 10 ft to the ground. Employee #1 was killed in t he fall. The original narrative did not mention the injuries sustained by the ot her employee.
