EMPLOYEE KILLED DUE TO FRACTURED NECK FROM FALL
Investigation abstract
A stock attendant was washing a portable tank over a grated 6-ft-deep pit. The p it was partially filled with waste water and waste chemicals from the washing pr ocess. The spigot of the tank became tightly wedged in one of the 1-in. by 4-in. grate openings. As the employee tried to dislodge the spigot, the grate he was standing became displaced and the employee fell into the small opening between t wo grates. As he fell through the space created between the displaced grate and the adjacent grate, his neck was caught and fractured. He was killed and was lat er found.
