EMPLOYEE INJURED WHEN FELL FROM A LADDER
Investigation abstract
Employee #1lost his balance while climbing down a ladder and fell 8 ft from the ladder. Employee #1 sustained injuries and was hospitalized. It was noted that t he company has an effective safety program, and that Employee #1 has had numerou s safety instructions on ladder safety, and that he should have recognized the u nsafe condition. However, it was noted that the ladder had one of its two rubber feet missing and the second one was loose. Moreover, the ladder was not tied in at the top.
