EMPLOYEE KILLED IN FALL FROM SCAFFOLD
Investigation abstract
Employee #1 was working on a two-point suspended scaffold in excess of 100 ft ab ove the ground painting the outside of a hotel. Employee #1 fell off the scaffol d to the concrete area below. Employee #1 died of massive internal injuries. Fal l protection was provided but not worn.
