Employee crushed when pulled into a revolving wire spool
Investigation abstract
On December 12, 1989, an Employee was working around lubrication area of die box es while machine was in operation. Wire was being drawn through the die boxes wh en the wire snagged an employee and pulled him into the revolving wire spool. Th e employee suffered fatal crushing injuries to his neck and chest area resulting in his death.
