Employee killed under stack of particle boards
Investigation abstract
At approximately 9:30 p.m. on February 1, 1990, Employee #1 was writing the iden tifying markings with chalk on the end of a stack of particle boards after placi ng himself into a 17 in. area between the end of the roll conveyor and the shutt le conveyor. Without any advance warning, the stack of particle boards automatic ally moved from the roll conveyor onto the shuttle conveyor, striking Employee # 1 in the waist. He was pinned underneath the stack of particle boards measuring 9 ft high by 24 ft long and 50 pieces high of 3/4 in. each. The boards weighed a pproximately 10,000 lb. Employee #1 was killed.
