Employee amputates three fingers in mat machine
Investigation abstract
Employee #1 was working where a mat exits at the end of a mine mat machine; this ng amputated. machine punches, forms, and straightens mine mats. A mat became stuck after par tially exiting and the employee turned off the machine to locate the problem. He reached over to the control panel to turn on the machine, intending to pull on the end of the mat to help it out, when he inadvertently pushed the start and re verse buttons at the same time. The fingers on his left hand were sticking down through a slot that had been punched in the mat and when he pushed the buttons, the mat reversed, pulling his hand back into the machine.
