Employee suffers fractures in fall from catwalk
Investigation abstract
At about 1:10 a.m. on November 14, 1989, Employee #1 was installing a video came en left wrist. Employee #1 was transported to the hospital by ambulance. ra in the ceiling area above the sales counter. Employee #1 was on an unguarded suspended catwalk above the ceiling tiles. While reaching for an electrical cord , he lost his balance and fell from the catwalk through the ceiling tiles, onto a display rack, then to the tiled concrete floor. The catwalk was about 8 to 10 inches above the ceiling tiles and the display rack was about 8 feet from the ce iling; the ceiling to floor distance was about 11 feet. The accident was witness ed by another employee who was assisting in the installation. The injuries susta ined included a pelvis broken in three places, a broken T-7 vertebra, and a brok
