Employee injured in fall from mixer

Investigation abstract

Employee #1 was standing atop a 1,000 lb dough mixer, washing it with a water ho se through the opening in the top. He lost his balance and fell about 7 feet to the floor, fracturing his right wrist and cracking several ribs on his right sid e.
