EMPLOYEE KILLED BY CRUSHING DURING MACHINE REPAIR
Investigation abstract
Employees working for Southern Machinery Repair were setting up a rebuilt 200-to n Verson Press at the company's WCI Range Division Plant. Employee #1 had been s ent to obtain oil for the press by his Leadman. While Employee #1 was gone, the Leadman and a Millwright were going to cycle the press to run some amp tests. Em ployee #1 returned with the machine oil and was ordered to climb onto the ram du ring a pause in the testing, to add the oil. An operator at the front of the mac hine activated the press from the front side, and Employee #1 was caught on the backside of the press between the ram and the top of the press crown. He was kil led as a result of being crushed.
