EMPLOYEE LOSES LEFT THUMB-LEAD PLATE STACKING MACHINE

Investigation abstract

At 2:35 p.m., on August 28, 1989, Employee #1, a utility man and machine operato r, lost his left thumb when he placed his left hand in the strap of the lead pla te stacking machine. He was cleaning around the stacker machine. The two machine s were connected by convey limit switches that keep stacks of lead plates in wai ting to be placed in the machine. As he placed his left hand in the point of ope ration, the operator picked up a stack of lead plates. This caused the machine t o start. He caught his left thumb on the machine. The employee's left thumb was amputated, and he was transported to the local hospital.
