On 1/22/15 at approximately 8:00 a.m., a worker was working on an overhead shop heater that was mounted on the wall at approximate shoulder height. He was standing on the floor during the maintenance activity and had pliers in his hand. He lost his footing and fell on the same level. He fractured his right hip and was hospitalized as a result.
