An employee fell approximately 12 feet from the top of a DOT water tank at the Naval Shipyard on February 2, 2015. The employee had been filling the tank with waste water from a naval vessel. They had just completed the job when it started to rain. The supervisor or team leader asked the victim to climb up and close the top hatch. The victim fell off, landed head first on the asphalt, and required hospitalization.

The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.
