Final narrative
On or about 2/6/15 at approximately 3:25 p.m., a worker was operating metal roller equipment. The worker turned on the switch and began feeding the sheet metal into the roller. Once a piece of sheet metal is placed into the rollers the machine begins the process of rolling the sheet metal into a cylinder form. The worker then began feeding another piece of metal through the rollers. At the same time, he reached over and removed the previously formed cylinder. The worker's glove was caught by the roller, which pulled his hand into the machine. The machine crushed his right hand.
