An employee was operating a slitter machine and grabbed the latch. The latch is stopped at 90 degrees by a safety pin, however the safety pin was worn out and it did not stop the latch. The latch continued past the 90 degree angle with the employee still holding it with his left hand and made contact with the metal box that houses the slitter's major bearings. Once the latch made contact with the metal box it crushed employee's left little, ring, and middle fingers. The little finger was amputated at the hospital.
