Final narrative
A temporary employee was running normal production operations on a stamping press. The employee hand-fed extruded aluminum pieces into the point-of-operation with actuation controlled by two-hand controls located at shoulder height. The employee cycled the press, and once the cycle completed he removed the punched part. Subsequently, he put new blank into the point-of-operation and at that time, without manual actuation, the ram drifted from its top-stop and came down while both of his hands were in the point-of-operation. Both hands were crushed by the ram.
