Final narrative
On December 3, 2015, at approximately 1:45 pm, an employee fell from an 8-foot fiberglass stepladder. The employee was installing an exterior light on the side of a building using a hammer drill while standing on the stepladder. The employee fell from the ladder and struck the concrete driveway, fracturing both wrists. The employee was hospitalized for surgery.

A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground.
