An employee was operating an industrial truck to move materials that were frozen to the ground. When the materials broke free, they struck and injured another employee that was on break sitting at a picnic table. The employee was pinned between the table and a concrete wall suffering broken ribs and a punctured diaphragm.
A second incident occurred on Jan. 19, 2016. An employee was exposed to carbon monoxide as it was pulled in and accidentally vented through an electrical panel due to a faulty heater unit. The employee was hospitalized and treated.
