Forterra Pressure Pipe

An employee was operating a pipe coating machine, ensuring that the product (steel pipe) was being carried through the machine as close together as possible. When he attempted to adjust a pipe, his shirt sleeve was caught between the pipe and a passive tire, pulling his right arm into the machine. His right forearm was fractured and had internal bleeding, requiring surgery. The machine was not guarded or locked out at the time of the incident.
