Final narrative
An employee took the cover off a chain and tried to grease a bearing. The PTO was engaged and the chain was moving at the time. When the employee put one pump in, the chain caught his glove, sending his fingers in through the sprocket and chain. His left thumb was partially amputated, and his index, middle, and ring fingers were lacerated and crushed.
