Final narrative
An employee was using a spud wrench to loosen bolts on a steel form panel. The nut was on very tightly, so he placed his foot on the wrench to loosen it. He lost his balance and the point of the wrench went up through the sole of his work boot, puncturing and fracturing his right foot.
