On or about June 27, 2016, an employee incurred a fingertip amputation of the little finger and laceration of the ring finger on her right hand. The employee was operating a laminating machine when the machine stopped. The employee tried to get the machine started by adjusting the belt. When she touched the belt, the machine started up, catching her fingers.
