Final narrative
An employee was using a forklift to unload a pallet from the top rack (approximately 15 feet) of an industrial storage rack. The employee stepped off of the forklift to inspect the contents of the pallet he had just moved. At this time, three separate cardboard boxes of rubber hoses fell from the top rack. While backing away, the employee suffered a scalp laceration (from being struck by the boxes or from tripping and falling).
