Final narrative
An employee placed a pallet of material on a platform with a forklift and then backed the forklift up to the platform gate. When the employee reached out of the lift to shut the gate, the index and middle fingers were crushed between the gate and the forklift cage. The employee's middle finger was partially amputated.
