Final narrative
On August 25, 2017, an employee was tasked with using a crimping machine to attach terminals to the ends of wires. The employee was able to fit their finger into the area where the terminal is crimped onto the wire end. The employee was attempting to push the wire into the holder to be crimped when the foot pedal was engaged activating the machine and amputating the employee's left index fingertip. The machine was not guarded at the time.
