Final narrative
An employee was changing a motor and gearbox on a thermoforming machine. During the process, the shaft stayed up in the platen. While he was positioning the motor using his hands, the shaft released, causing the table to go to the lowest position and the arms to raise up. This created a scissor-like motion, which amputated the employee's right ring fingertip.
