Final narrative
Employees were using a pallet jack stand to move several pallets of stacked ceramic tile weighing approximately 2,200 lbs. from one end of the floor to the opposite end when the tile hit a hole and the bundles of tiles began to wobble. The injured employee was standing on the side of the tile bundles and was attempting to balance them when they eventually tipped over and fell on him. He was pinned beneath the floor tile and a wall, sustaining a displaced open wound fracture to his left foot and ankle that required surgery. He also sustained minor injuries to both of his legs and scrapes, cuts, and bruises on other parts of his body, including a laceration to his head.
