An employee drove a road-rail material handling truck to a fabrication business/shop to transport a rail cart. The road-rail truck was equipped with a boom lift, and the employee sat in the boom operator seat and used the boom to lift the rail cart off the truck. While lifting the rail cart off the road-rail truck, the boom over-extended, which caused the road-rail truck to lean. The rail cart then fell off the boom, causing the road-rail truck to rock and the employee to fall out of the boom operator seat to the ground below. The employee suffered a broken left elbow, broken right knee, and minor brain and liver bleeding, which required hospitalization. A seat belt was not worn at the time.
