Final narrative
An employee was working from an extension ladder installing lights on the outside of a building. The employee and the ladder fell approximately 10 feet landing on a concrete sidewalk. The employee suffered a dislocated right shoulder, left distal wrist fracture, left non-displaced radial head fracture (arm), multiple right pelvic fractures, a concussion and laceration.

A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground.
