On October 16, 2018, at 3:45 PM, a temporary employee reached through the mast to scan a pallet that was on the forks of a powered industrial truck. While the employee was leaning forward to scan the pallet, the lift mast was engaged, crushing the employee's hand between the mast and fixed point of the lift and breaking bones.
