An employee was installing girt on the metal roof (30 degrees) of a newly built greenhouse from a scissor lift. He got off the scissor lift to nail down the top of the girt, then fell 26 feet through the roof to the ground, breaking his left leg and injuring his neck. He was hospitalized. Fall protection was not being worn at the time of the incident.

While changing the filters, the employee stepped on the cover. The cover gave way and the employee fell through the grating, landing approximately 26 feet below on a concrete floor. The employee sustained a broken arm, a broken rib, and a punctured lung.
