An employee was cleaning a theater and walked up the ramp to the projection booth to turn down the volume. When he left the projection booth, he jumped over a guardrail (6 feet) and impacted the floor, suffering neck and back pain. He was hospitalized with a neck injury.

An employee was picking up a car from a fenced-in area when they became locked inside. The employee climbed the 7-foot fence, jumped off onto the concrete surface, and sustained bilateral ankle fractures.

CSC Holdings, LLC

An employee was descending an extension ladder that was set up on a cable strand at approximately 18 feet. When the employee was about halfway down, the ladder started to shift to the left and the employee jumped off the ladder to the right. The employee landed on a concrete sidewalk and their right ankle was fractured.

An employee was practicing an acrobatic act and jumping from one swing to another when she fell approximately 10 feet to the wood floor below, resulting in a lower lumbar fracture. Fall mats were in place at the time of the incident, but the employee fell outside of the mats.

A driver was inspecting an empty container for damage. While the driver was inside the container, a stacker operator started lifting the container, which was still attached to the chassis with four twist locks. The driver, upon feeling movement, jumped approximately 6 feet out of the container onto the concrete ground and broke his heel in two places.
