An employee pulled eight cases of orange juice inside bread baskets weighing approximately 170 pounds across the dock from the staging area to a van. He was trying to load the cases of juice onto the van by hand when the baskets became stuck on the dock going into the van. He pulled the bread baskets and they started to tip over onto the van. As he tried to catch the falling baskets, his right hand was caught between the van floor and bread baskets. He pulled his hand out, resulting in a right middle index fingertip amputation.
An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.
An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.
