Final narrative
On December 12, 2018, at approximately 6:53 a.m., an employee noticed that a piece of scrap metal from the previous cycle had become lodged in the die area of the blanking machine. While attempting to remove the piece of metal with his hands, the employee slipped on a separate piece of scrap metal that was on the floor. He lost his footing and contacted the foot pedal. The blanking machine cycled while his left thumb tip was in the die area. The thumb tip was amputated at the thumbnail area. The machine was not guarded at the time.
