On January 17, 2018, at approximately 3:00 p.m., an employee was picking up ventilation duct work on the outside retaining wall of an outdoor grain pile. A second employee was operating farm equipment inside the grain retaining wall to move corn seed. As the second employee was moving seed, a piece of ice on the tarp covering the pile was dislodged and slid down the tarp, striking the retaining wall and sending pieces into the air. One of the pieces of ice struck the first employee on the right side between the rib cage and hip. The employee was hospitalized due to internal muscle bleeding.
