Final narrative
An employee was cleaning an elevator shaft pit prior to installation of elevator doors. The employee began descending a step ladder into the elevator shaft pit when the employee fell from the ladder onto a steel pipe at the bottom of the pit, fracturing the right femur. The employee was hospitalized and had surgery. The step ladder legs were not set and locked at the time of the incident.
