An employee was using a hoist to lower round stock into a module after a grinding operation. He was controlling the hoist pendant in his left hand and guiding the lifting sling with his right middle finger. The sling was being pulled up by the hoist, its loose end drawn through the bars. The bars were not separated at each end to have space for the sling to move freely and they rolled, pinching his finger. The sling was then drawn through the bars with his finger still attached. His finger was trapped in the hole of the sling, causing a partial amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
