Final narrative
An employee was rigging a muck box on a crane. As the crane operator hoisted slack out of the rigging, one of the lifting eyes did not align. The employee attempted to hit the lifting eye with his hand to straighten it out when his left ring, middle, and index fingertips were crushed by the muck box lifting eye, resulting in an amputation.
