Final narrative
An employee was setting up a line for 14-inch PVC pipe production. As the PVC pipe came out of the extruder, it started to sag. The employee lifted the pipe with the left hand to align it and was controlling the remote with the right hand. When the tank came in, the employee's left thumb was caught between the alignment pin and the tank, resulting in a thumb tip amputation between the nail and first knuckle.
