An employee was conducting grinding operations on a flange weighing 265 pounds. The flange was supported by two metal saw horses. After the employee finished grinding on the top side of the flange, the flange had to be flipped over to grind the bottom side. A crane was used to lift and flip the flange. As the flange was being flipped over, it started to fall off one of the saw horses. The employee attempted to grab the flange. The employee's middle finger on his left hand was caught in between the flange and the saw horse. The employee was hospitalized for a compound fracture of the left middle finger that required surgery.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
