A joint of pipe that had an end hook was conveyed into the CNC machine. In order to process the hooked material, an employee decided to shim the pipe. He was holding a shim next to the pipe with his right hand while his left hand was resting on the chuck ring. He activated the foot pedal that engages the self-centering arms to align the pipe and clamp the jaws. Once the pipe was aligned, the jaws clamped on the pipe and the centering arms retracted, causing the employee's left middle finger to be caught in the chuck ring. He suffered a left middle fingertip amputation.
